Mountain States Pharmacy

We’re a pharmacy, a retail home supply store, and for patients who have been hospitalized at Johnson City Medical Center, Indian Path Medical Center, Franklin Woods Community Hospital, Norton Community Hospital and Johnston Memorial Hospital in Abingdon, Va., we’re a source of information and guidance to help you transition from the hospital to your home.

Our Coordinated Care Guide Team

In an effort to reduce readmission rates and improve the patient experience, Mountain States has developed coordinated care guide teams. These teams currently serve at six of our hospitals. Our coordinated care guide teams focus on patient care. They assist with various patient needs at discharge and work daily in collaboration with case management, social work, and the nursing staff to reduce the factors that could lead to a patient being readmitted. This includes assistance with medications and durable medical equipment that the patient needs along with other outpatient service lines.

The goal of the program is to reach every aspect of patient care and provide as many services to the patients and fellow team members as possible, with the intention of reducing readmission rates; increasing patient satisfaction; making transitions for patients, family members and fellow team members swift and convenient; and supporting the continuum of care.

With the help of our Mountain States Pharmacy locations, patients’ discharge medications are processed and filled at convenient locations for pickup. This makes it easier for patients to receive and be counseled on their medication, and also alleviates uncertainty about medication coverage. Coordinated care guides also assist in delivery and education for durable medical equipment (DMEs), in partnership with Mediserve, and they also offer assistance to hospice patients to help reduce their discharge time.This team can a big difference to our patients as they leave the hospital, by helping them get everything they need to continue a full recovery and prevent readmission.

As the coordinated care guide team grows, additional services will be added to more locations. The coordinated care guide team serves Johnson City Medical Center, Niswonger Children’s Hospital, Indian Path Medical Center, Franklin Woods Community Hospital, Johnston Memorial Hospital and Norton Community Hospital.

Compounding

What is Compounding?

  Compounding is the preparation of a customized medication used to solve a patient’s problem by meeting his or her specific prescription needs.  Compounding pharmacists work with both prescribers and their patients in order to develop the most appropriate therapy for each person.

How can compounding Help Me?

Compounding allows a pharmacist to customize medications into a variety of different dosage forms, strengths, and flavors.  Here are some examples of how our compounding pharmacists can help you.

Veterinarian Compounding

For “pet parents” owning a pet includes veterinary care and possibly the need for medications.

Veterinarians work with many different animal species, large and small, but often face a handicap in treating their patients. Many of the medications they need to optimize an animal’s care are not available in the form and strength appropriate for your pet.

Most veterinarians have learned to turn to a compounding pharmacist for help when their options are limited.

As a compounding pharmacy, Mountain States Pharmacy knows that one size does not fit all when it comes to health.  Hormone Replacement Therapy is a natural way to treat premenopause and perimenopause.  Changes in your natural hormone levels can cause uncomfortable symptoms during premenopause and perimenopause; correcting this hormone imbalance with plant derived hormone replacement therapy is a great way to help you feel your best.

Pain Management Compounding

Chronic pain can have a wide variety of causative factors therefore it requires a multidisciplinary approach and uniquely customized treatments to meet the needs of each patient.  Treatment may involve a combination of medications that possess pain-relieving properties.  This may include muscle relaxants, NSAIDS, anesthetics, anticonvulsants, antidepressants, and NMDA receptor blockers.

Topical and transdermal creams and gels can be compounded to provide high concentrations locally at the site of pain (i.e. degenerative joint disease, heel spurs).  Topical application of these agents often eliminates or significantly decreases the side effects associated with oral administration.

Our staff works in conjunction with the medical prescriber and the patient to customize medications that meet the specific needs of each individual.

Doctors and parents know that giving children medications can present several challenges. At Mountain States Pharmacy we provide everything from flavoring medications, to enhance taste, to providing individualized strengths and dosage forms. Immunizations

Wound Care

Wound care is an essential part of Mountain States Pharmacy, and our pharmacists understand there is no one-size-fits-all solution. We carry both medicated and non-medicated wound care supplies for everything from a simple scraped knee to a neuropathic ulcer. Each wound presents its own individual challenge, but our experienced team is ready to help you no matter where you are in the healing process.  We work closely with wound care clinics to provide a customized wound care solution to meet our patient’s needs.

Infusion therapy is when a drug is administered intravenously via a peripheral or central vein.  Mountain States Pharmacy provides these medications to patients in the home setting either to prevent hospitalization or allow patients that are stable to come home from the hospital.

  Diseases commonly treated with Infusion therapy include but are not limited to:

  • Infections unresponsive to oral antibiotics
  • Cancer and cancer related pain
  • Dehydration
  • Gastrointestinal disease or disorders preventing normal functioning of the gastrointestinal system
