Diabetes Care

The Diabetes Care Center is an outpatient department of JMH that provides medical management of diabetes and diabetes-related conditions for patients aged 12 and up. Patients are seen by appointment and are referred by primary care providers, Emergency Department physicians, hospitalists, and patients can self-refer as well. Care is provided with a team-based approach with the patient as an integral member of the team. Consultation services for inpatient glycemic management are also available upon request. 

The Diabetes Care Center also offers a comprehensive Diabetes Education program that is recognized by the American Diabetes Association. People of all ages with diabetes are seen by appointment and a referral from a primary care provider is required. Following comprehensive assessment of the patient’s diabetes education needs and interests, an individualized educational plan is developed in one or group-based educational sessions. Education topics include, but are not limited to, self-monitoring of blood glucose, foot care, exercise, meal planning, diabetes medications, continuous glucose monitoring, insulin pumps. A monthly support group for people with diabetes is offered March – October.

Goals for Patient Care

Our goal is to provide excellent comprehensive diabetes care. We want people with diabetes to get the medical care they deserve and the education they need so that they are able to effectively manage their diabetes. We monitor our patient’s clinical outcomes as compared to national standards, diabetes-related knowledge and skill, diabetes self-care behaviors, and patient satisfaction to ensure that we are meeting our goals for patient care.

HRC Education Program

No physician referral needed. Education topics include, but are not limited to, self-monitoring of blood glucose, foot care, exercise, meal planning, diabetes medications, continuous glucose monitoring, insulin pumps.

Glucommander – inpatient only

  • Software program to help with glycemic control.
  • Insulin dosing software that will help to better manage patient’s diabetes care.

Mountain States chosen to lead new, innovative program aimed at improving the health of communities in Virginia

Life-saving sepsis initiative by Mountain States, Johnston Memorial Hospital team wins national award
