David "Rick" Moulton, MD
Dr. David “Rick” Moulton serves as the medical director of clinical integration for State of Franklin Healthcare Associates (SoFHA).
SoFHA is a physician-owned, multi-specialty organization with 127 providers and 600 team members. Moulton served on the board of governors for SoFHA from 2004 until 2010 and was chairman of the board from 2005 until 2010. As the medical director for clinical integration and chair of the SoFHA clinical transformation committee, he has led the organization to level 3 Patient-Centered Medical Home (PCMH) recognition by the National Committee for Quality Assurance (NCQA).
In addition to his roles within SoFHA, Moulton is co-medical director of Qualuable Medical Professionals, a physician-led accountable care organization with 722 members serving Northeast Tennessee and Southwest Virginia.
Moulton has been affiliated with East Tennessee State University’s James H. Quillen College of Medicine since 1992. He directed an outpatient continuity clinic (teaching site) for ETSU at Johnson City Internal Medicine beginning in 1999 and is currently a clinical associate professor with ETSU.
Moulton is certified by the American Board of Internal Medicine and has been part of the SoFHA team at Johnson City Internal Medicine since 1995. He received his Bachelor of Science degree from The Ohio State University and his doctor of medicine degree from the University of Cincinnati School of Medicine. He completed his internship, residency and chief residency at the University of Maryland.
Dr. Moulton will also serve on the quality committee of the board.
Dr. Moulton is married to Linda, and they have two children – David, who is a fisheries ecology PhD student at University of British Columbia, Vancouver, Canada; and Marie, a third-year medical student at ETSU Quillen College of Medicine. In his spare time, Rick enjoys swimming and fishing.
